Gentazaf-Z 80 mg 2ml Sol for Injection 5 Amp, Gentamicin

Gentamicin is a broad-spectrum aminoglycoside antibiotic. Acts on negative aerobic bacteria, including Enterobacteriaceae, Pseudomonas and Haemophilus. It also acts on staphylococci (Staphylococcus aureus and Staphylococcus epidermidis) including penicillinase producing strains, has very limited activity on streptococci. It has no activity against anaerobic bacteria.

GENTAMICIN Injection is indicated in infections caused by susceptible organisms:

Abdominal infections.

Skin and soft tissue.

Gastrointestinal infections.

Biliary infections.

Genitourinary infections including complicated and recurrent infections.

Bone infections.

Infections in burns.

Other infections: Meningitis, septicemia, peritonitis, listeriosis, plague, pneumonia (Klebsiella, Pseudomonas) inguinal granuloma.

CONTRAINDICATIONS: Known hypersensitivity or serious toxic reactions to gentamicin or other aminoglycosides.
DOSAGE AND ADMINISTRATION: gentamicin sulfate injection may be administered intramuscularly or intravenously. You must obtain the patient's weight before treatment, for the correct dosage.

The dosage of aminoglycosides in obese patients should be based on an estimate of lean body mass. It is advisable to limit the duration of aminoglycoside therapy a short time.

It is recommended to determine serum concentrations of gentamicin, to ensure adequate but not excessive levels. After intravenous or intramuscular injection of gentamicin, two or three times a day, the maximum concentration, measured between 30 minutes to 1 hour after administration, is expected to be between 4 to 6 mcg / ml.

With the administration of a daily dose can be anticipated but transient high peak concentrations. In all schemes the dose should be adjusted to avoid prolonged concentrations above 12 mcg / ml. You should also avoid higher peak levels of 2 mcg / mL, measured just before administering the next dose. To determine if a serum is suitable for a particular patient, one must consider the sensitivity of the causative organism, the severity of infection and immune status of the patient.

The duration of treatment for all patients is 7 to 10 days. Complications in infections may require longer periods of therapy.

For patients with serious infections and normal renal function: The recommended dose of gentamicin sulfate injection is 3 mg / kg / day administered in three equal doses every 8 hours, or two equal doses every 12 hours or a dose daily.

In patients with life-threatening infections: They can administer a dose as 5 mg / kg / day, divided into three or four equal doses. This dosage should be reduced to 3 mg / kg / day, as soon as clinically indicated.

In patients with urinary tract infections, particularly if chronic or recurrent, and without evidence of renal failure, weighing 50 kg or more, gentamicin can be administered intramuscularly at a dose of 160 mg once daily for 7 to 10 days.

For adults weighing less than 50 kg: single daily dose should be 3.0 mg / kg of body weight.

When possible, you should periodically determine the peak and minimum serum concentrations of gentamicin for treatment, to ensure adequate but not excessive levels of the drug.

When determining the peak concentrations after intramuscular or intravenous administration, the dosage should be adjusted to avoid prolonged levels above 12 mcg / ml.

When determining the minimum concentration (just before the next dose), dosage should be adjusted to avoid levels above 2 mcg / ml.

The determination of serum level appropriate for a patient in particular, considers the susceptibility of the causative organism, the severity of infection and immune status of the patient.
